How do I Qualify?
- Must be under the care of a physician who orders for home health
- Must be in need of skilled care
- Must be homebound
Homebound: This requirement is met if the patient leaves home infrequently with difficulty and taxing effort to do so. (doesn't mean you cannot ever leave your home)
